CareDriven supports case managers and patients by building a team of experts to support and coordinate care from day one.
Our #1 priority as clinicians is open communication with Case Managers and the other health care professionals involved in a patient’s care. Our team provides specific, relevant and appropriate patient reporting that is personalized to your criteria and expectations. Attendant Care
One-on-one hourly attendant care is available from 4 to 24 hours a day, with multiple services provided. As the leader of the care team, an RN designs and manages an individualized plan of care with input from the client and their family. The RN also visits regularly to ensure the plan remains appropriate.

Our trained and experienced home health aides provide care to help support your #1 goal — helping your client get back to their best state of wellbeing as quickly as possible.
Skilled Care
When a client’s condition requires the training, education and experience of a home care skilled professional, we have registered nurses and licensed practical nurses on staff in addition to physical therapists, occupational therapists, and speech-language pathologists to aid in recovery. Our skilled professionals will work closely with, and under the direction of the client’s physician, to develop a plan of care to meet the short- and long-term goals of care.
