Enabling residents to get more out of their daily lives.


Wellness services are scheduled services to assist residents with personal care or ac vi es of daily living as well as ensure their safety as they age in place. Our Wellness Services are managed by a Wellness Nurse Manager with years of clinical and management experience. The Wellness Nurse Manager develops a plan of care a er assessing the resident and discussing his/her needs with the resident and/or family. The Wellness Nurse Manager oversees the services provided by our Resident Assistants who have been carefully selected and specially trained for their responsibili es.

Wellness Services may range from à la carte to customized service plans as well as unplanned services. Wellness Packages and customized service plans bundle together some of the residents’ most-needed services.

À LA CARTE SERVICES

Assistance with Activities of Daily Living and special services are provided by Resident Assistants with Registered Nurse Supervision

à la carte services include but are not limited to the following. A resident may sign up for one or more services:
  • Stand by or Complete Assistance with Dressing and Grooming
  • Stand by or Complete Assistance with Showering
  • Meal/Activity Escort/Transport
  • Continental Breakfast Preparation
  • Resident Checks Every 2-3 hours
  • Medication Reminders
  • Toileting Assistance
  • Urinary Bag Emptying and Changing
  • Exchanging O2 tanks
  • Blood Pressure Checks
  • Blood Glucose Checks
  • Daily Weight Checks
  • Brace/Ortho CS on/off
  • Assistance with hearing aide
  • Support Hose On/Off

When a resident’s needs go beyond Wellness Services, CareDriven has the experience, knowledge, and skill to provide One-on-One continuing care from 4 to 24 hours a day. One-on-One care is often required when a resident is unsafe to be left alone for any length of time or not able to independently care for self. Caregivers are matched for compatibility with the client by the managing Nurse based on the client’s needs, caregiver’s skills, and personality.

Medication Management

  • Communication with physician to obtain/verify orders
  • Coordinating medications among pharmacy, healthcare providers and family
  • Monitoring and oversight of narcotic medications
  • Assessment and reporting of side effects to physician
  • Medication Set Up
  • Medication titration to physician ordered parameters

Care Management

This service is especially beneficial for residents/families that are not adept at naviga ng the healthcare system or are not available to get directly involved with the intricacies of the resident’s health care needs. It involves the procurement and coordina on of the resident’s care with a physician, other outside health care providers and families.

Complimentary Services

The following additional services will be provided to residents, free of charge.
  • Assessment of potential resident to determine level of independence and need for Wellness services for resident’s safety and well-being in his/her new home.
  • Follow up calls or visit to the hospital/healthcare facility to coordinate resident’s care at discharge and ensure a smooth transition back to the community.
  • Assessment of residents for changing care needs and/or assistance with activities of daily living as requested.
  • Referral to a Medicare certified agency as necessary for covered home care visits
