Professional Nursing Services

When a client’s condition requires the training, education and experience of a skilled professional, we have Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists and Speech Language Pathologists on staff to provide the care. Our nurses will work closely with, and under the direction of the client’s physician, to develop a plan of care to meet the resident’s needs.

Medication Management

  • Communication with physician to obtain/verify orders
  • Coordinating medications among pharmacy, healthcare providers and family
  • Monitoring and oversight of narcotic medications
  • Assessment and reporting of side effects to physician
  • Medication Set Up
  • Medication titration to physician ordered parameters

Care Management

This service is especially beneficial for residents/families that are not adept at naviga ng the healthcare system or are not available to get directly involved with the intricacies of the resident’s health care needs. It involves the procurement and coordina on of the resident’s care with a physician, other outside health care providers and families.

Complimentary Services

The following additional services will be provided to residents, free of charge.
  • Assessment of potential resident to determine level of independence and need for Wellness services for resident’s safety and well-being in his/her new home.
  • Follow up calls or visit to the hospital/healthcare facility to coordinate resident’s care at discharge and ensure a smooth transition back to the community.
  • Assessment of residents for changing care needs and/or assistance with activities of daily living as requested.
  • Referral to a Medicare certified agency as necessary for covered home care visits
