One-On-One Care consists of hourly care day or night. Every home care assignment begins with a Registered Nurse evaluating each client and determining what services would meet his or her needs and designs a plan of care with the client and/or loved ones. That plan guides the staff to provide the required care, enhance the individual’s quality of life, as well as ensure client’s safety and well-being in his/her own familiar residence. The Registered Nurse leads the care team and provides supervisory visits to ensure that the plan of care continues to be appropriate for the client. One-on-One care Caregivers are matched for compatibility with the client by the managing Registered Nurse based on the client’s needs, caregiver’s skills, and personality.

For a patient in a health care facility anticipating discharge, a Registered Nurse may assess the patient at the facility and make a home visit as well. Proper planning and coordination of the patient’s care at the time of discharge can help ensure a smooth transition back home.

People recover best when they’re in comfortable, familiar surroundings. We work hard to keep the injured, elderly, the ill, or the disabled living in the homes they love. Our trained staff provide services ranging from assistance with activities of daily living to skilled care for complex, catastrophic cases.

One-On-One Care consists of hourly care day or night to provide services such as but not limited to:

Home Health Aides

Our compassionate, reliable, trained and experienced Nurse Aides provide assistance and companionship to keep you or your loved one safe. They assist with Activities of Daily Living to maintain your or your loved one’s well-being and independence or provide complete care for clients who are unable to care for themselves due to illness or injury. Our nurse aides have gone through a 75-hour Nurse Aide Training course with a minimum of 6 months experience.

Skilled Services

When a client’s condition requires the training, education and experience of a skilled professional, we have Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists and Speech Language Pathologists on staff to provide the care. Our licensed professionals will work closely with, and under the direction of the client’s physician, to develop a plan of care to meet the client’s needs.
